I believe that it's the one that is available only to supporters at Startrekguide: http://startrekguide.com/community Once you've signed up and donated you should be able to see their supporters section where you can download the mod.

If it doesn't have to be exactly like that one you can try these two mods together:

with this for a way to edit the rules page from the ACP:
http://www.phpbb.com/customise/db/mod/faq_manager

Just remember that with that mod it requires you to set permissions on each group you want to see the rules, including administrators/moderators.

The FAQ manager is just a drop in and then run it's "faq_manager.php" file and delete that file. The Rules page from phpbb.com DB, isn't that hard to install and then run "install_rules_mod.php" and delete that file. The edits aren't really that massive.
