I use to go to this one forums site and they had a Graphic Request Form that when you requested Images made that you would fill out when you made a new thread. And I was wondering if I could get it made for my phpbb forums. Below is a Image of the "Form" from there site and a url to the code on there site.

Note: You don't have to use there coding I just thought I would supply there coding so you can see how they made it. And If you guys don't do request then that's ok.
