Help with install

I recently upgraded a forum to phpBB 3.0.12; previously it was using ajax_shoutbox. In upgrading the forum, I basically performed a fresh install and want to add the mchat MOD to it.
I have tried installing mchat manually and with AutoMOD; both times I can get the chat to appear on the index page and it appears like it is usable; however, when I try a test post in mchat, the circle keeps going like it is trying to update, then when I refresh the page a blank white page appears. What am I missing? (If I use AutoMOD, I have to update the constant file on my own).
