In case you are using some broken FTP client which uses [url=http://blog.phpbb.com/2011/08/23/the-dangers-of-ascii-mode/]ASCII mode[/url], then kindy either fix the settings or stop using it immediately.

I fixed all htm/html/css/js/php/.htaccess files using + bash script. If it still does not work, you are on your own since I have absolutely no clue how you managed to do this. Even stock unmodded files had the linebreaks screwed.

When your index consists of a single linebreak, it will not work... There are NOT supposed to be any whitespace chars before the opening tag!
