The ones that work.
Seriously, most of my favorite MODs are simple, things that seem obvious in hindsight. Like making the forum icons clickable. If you have ever watched a computer rookie using a phpbb forum, you know that one of the things they want to do is click on the forum icon (or the topic icon) rather than the forum name (topic title) in order to open the item.
Defaulting shadow topics to "off" is a highly simple yet effective change.
Providing a default title on post replies is an obvious yet non-default feature.
Being able to search titles only.
All of the above are MODs that I've written and have in the phpBB MOD-DB, but that's not the reason why I list them as my favorites, they are favorites because they make the system work better. They don't add flashy features, they just make it work better.
I also like Page Permissions I use it on every board I set up now. It's just too useful.
I like / admire the Cash MOD, there's a lot of good code in that one. The attachement MOD has to get a few votes.

XD If I could have only one I think it be the Edit Store. That Mod is a must have for dealing with users who edit their messages to try and get away with something they did. It saves edits of all versions of a post, and displays each in one page. Idiots can't get away with covering up what they have done ;P

First of very nice Topic, however there are few things, I would like to add:
1. Don't let it die, keep it updated with New MODS addon.
2. It would have been quite usefull for new members "who have migrated from IPB, and vBulletin to phpBB?". If the MODS had their download links.
